You also assume full responsibility for any personal injuries, property damage, death, frozen and domestic acts, representation, safety, service of any and all physicians, hospital, and/or tourism services. You agree that AMO, nor our owners, employees, affiliates, or agents will in no way be held accountable for the following: liabilities, property, injuries, costs and/or expenses associated with both domestic and/or foreign court costs, hospital costs, airfare costs, and overnight accommodations costs. AMO is not responsible for any of the information and/or representation of the links that are posted on our website. All participants that are traveling to any of the overseas hospitals will need to be in possession of a current and up to date passport during the time of travel. Please check with you domestic doctor to obtain a medical professional's consent to travel before applying for your passport. AMO would be pleased and happy to help with the acquisition of documentation that may be needed to travel. Close estimate pricing will be provided after medical transcripts have been submitted to the doctor and treatment has been agreed upon. Prices may change depending on inter-operational discoveries. Depending on the healthcare facilitator, the method of payment may be required before the procedure. If the healthcare facilitator does decide to collect payment before the treatment, you still have the option of not proceeding with the treatment, and will receive a full refund of the payment that was collected up front. Should you choose to have your surgical procedure performed at one of the eight JCI-affiliated hospitals, only a few steps are necessary:

Contact AMO via email, phone, or fax to inform us of what medical procedure you are interested in. Once we receive this information, AMO will work with you to find the best facility and healthcare physician for your needs.
AMO will assist you with all of your travel arrangements (airfare, transportation to/from airport, hotel, and hospital, hotel reservations, passport, etc.)
AMO will also assist in acquiring all necessary medical documentation including, but not limited to the following:
- 'Fitness to Fly' Certificate (if medically advised)
- Copies or originals of all recent medical reports, including a list of current medications
- Digital or electronic copies of scans (i.e. MRI, CT, etc.)
- Any recommendations or referral notes from your doctor
Once you have arrived at your destination, a representative from AMO or from our partnering hospitals will pick you up at the airport, and drop you off at either your hotel or the hospital (depending on what you have requested or made arrangements for). At the hospital, you will be registered under the physician that AMO/partnering hospital recommended, and which you consented to.
Payments need to be made to our partnering hospitals under their terms and conditions. They can require payment in full prior to the procedure, or choose to work with the patient on a payment plan. Hospitals accept travelers' checks, foreign currency, and credit cards (Mastercard, Visa, and American Express). The patient will be provided with a receipt upon payment, and an itemized bill will be provided upon discharge.
